Membership Fees cover enhanced access and non-insurance concierge services only. Membership is not health insurance and does not replace your insurance coverage. Membership Fees are not reimbursable by Medicare, Medicaid, or any health insurance plan and must not be submitted to those programs.
Membership fees are due at onboarding and are billed quarterly unless option for a discounted annual plan. Membership is non-cancellable and non-refundable.
Standard clinical services (e.g., evaluations, medication management, labs, imaging, procedures, and hospital care) are billed separately to the Patient or the Patient’s insurer according to coverage. After-hours/urgent messaging is for non-emergency matters only and is not a substitute for emergency care.
Membership fees may qualify for FSA/HSA/HRA use, but the Practice makes no guarantee. If you request documentation for reimbursement, it may be provided, but reimbursement is not assured. Patients remain responsible for any clinical charges billed to insurance or paid privately.
